Where do I go to enroll?
In Person Option:

Print and complete the enrollment packet for your student's school

Drop off enrollment packet with the required documents at the east end of the Administration Office, located at 900 W Coctaw, Chickasha Oklahoma 73018.

Administration Office hours are Monday-Friday 8:00 a. m. to 4:30 p.m.

Where can I get a copy of my child's birth certificate?
Certified copies of birth certificates can be obtained from the vital records division in the state where the child was born. For a child born in Oklahoma, visit the Vital Records Service Room at the Oklahoma State Department of Health, 1000 NE 10th St., Oklahoma City. The office is open Monday through Friday between 8:30 a.m. and 4 p.m. For more information or for instructions on obtaining a birth certificate by mail or to order online, call 405-271-4040 or visit https://oklahoma.gov/health/birth-and-death-certificates/birth-certificates.html.
For a child born outside of Oklahoma, the National Center for Health Statistics provides a list of Vital Record agencies.

Where can I get my child's immunization record?
Acceptable immunization documents include a record provided by a licensed physician or a public health authority. These documents must include dates and a signature, or stamp of the physician or health department official.
Exemptions to state immunization law may be granted in accordance with state law and State Department of Health requirements. For more information, please visit http://imm.health.ok.gov

What can I use as proof of residency?
Acceptable proofs include: Current utility bill (gas, water, electric), rental agreement, warranty deed statement, or mortgage statement. Telephone, Internet, cable bills, checks and drivers' license will not be accepted as proof of residency.

What are the LEARNING PATHWAY options for the 2021-2022 school year?:
TRADITIONAL LEARNING PATHWAY
Students will participate in a traditional school setting with a full-day schedule. Remote learning days will be utilized in the event of inclement weather or exposure/illness resulting in quarantine.

- All courses/classes taken at school (traditional setting)
- In-person instruction
- Traditional academic school day
- Will utilize Google Classroom for smoother transition to remote learning days (as needed)
- Will have built in virtual learning days for students and staff to work together on on-line instruction
- Will transition as needed between in-class and remote learning days
VIRTUAL LEARNING PATHWAY
The virtual learning pathway is a complete online virtual learning experience for students.

- Students must complete application and meet eligibility requirements to participate in the virtual learning pathway.
- Student learning plans will be directed an monitored by CPS teachers
- Tutoring, assistance, and assessments could be given at an agreed upon location/time
- Classes do not require regular on-campus attendance
- Requires a semester commitment
- Parents and students must attend orientation meeting at beginning of year.
- Meals (breakfast & lunch) will be available to all students enrolled in this pathway.
